Patient Health Advocate

Job Summary

To support a growing care team, the full-time Patient Health Advocate will engage with recently hospitalized complex renal patients, coordinating care and facilitating transitions across various healthcare settings while working remotely.

Key responsibilities

• Conduct outreach to recently hospitalized members to engage them in the Somatus program and connect them with care resources
• Coordinate care by serving as an advocate and resource for patients, their families, and healthcare providers
• Assist patients in scheduling follow-up appointments post-discharge to ensure effective transitions of care and minimize readmissions

Required qualifications

• Experience working with Medicare, Medicaid, or Special Needs populations
• High school diploma or GED required
• Experience as a Medical Assistant, Licensed Practical Nurse, Engagement Specialist, or Community Health Worker
• Effective written and verbal communication skills demonstrating respect and cultural awareness
• Proven experience engaging patients in making healthy behavior changes
