Job Summary

A company is looking for a Patient Navigator - Population Health.

Key Responsibilities

• Establishes relationships with patients and serves as their primary point of contact
• Analyzes market data to strategize payor population identification and verifies insurance benefits for obesity treatment
• Coordinates patient appointments and assists with scheduling discharge appointments and prescription fills

Required Qualifications

• Bachelor's degree in Healthcare Management, Public Health, or related field, or an Associate's degree with two years of relevant experience, or a high school diploma with four years of relevant experience
• Preferred: Bachelor's degree in Healthcare Management or Public Health
• Three years of experience in the healthcare field, including one year in statistical data analysis
• Ability to navigate insurance pre-certification requirements and educate staff on payor regulations
• Experience in collaborating with social services for resource and financial management
