Job Summary
A company is looking for a Patient Navigator to support patients in navigating complex healthcare pathways and improve their healthcare experiences.
Key Responsibilities
- Provide responsive patient engagement through inbound calls and proactive outreach
- Assist with appointment scheduling, transportation coordination, and connecting patients to community resources
- Document patient interactions accurately and ensure compliance with HIPAA regulations
Required Qualifications
- High School Diploma or G.E.D. (required)
- Associate's degree in healthcare, social work, business administration, or related field (preferred)
- 2+ years of experience in customer service, healthcare navigation, or patient support (required)
- Experience in care coordination or healthcare access (preferred)
