Patient Navigator

Job Summary

A company is looking for a Patient Navigator for its Weight Management Program (Hybrid/Remote).

Key Responsibilities

• Perform direct outreach to patients for engagement and enrollment in the clinic
• Proactively resolve potential barriers for appointment or program enrollment and maintain KPI dashboards
• Document patient communication in the Electronic Medical Record (EMR) and provide general clerical support

Required Qualifications

• Education: High School Degree or GED; college-level study is a plus
• Experience: No prior experience necessary; customer service or patient care experience preferred
• Skills: Ability to work in a team and independently; creative problem-solving skills
• Preferred: Completed motivational interviewing training and experience in a healthcare or community-based setting
• Ability to adapt to change and work in a multicultural environment
