Job Summary

A company is looking for a Patient Navigator.

Key Responsibilities

• Provide clinical and clerical support, including triaging patient inquiries and routing calls to providers
• Ensure timely delivery of lab results and pre-visit preparations for patient appointments
• Assist care team in managing health needs of patients and facilitate social service referrals for vulnerable populations

Required Qualifications

• High-school level education or equivalency
• Medical Assistant Certification or equivalent, or at least 2 years of clinical experience in an outpatient setting
• Spanish fluency preferred
