Job Summary
Patient Navigator is a full-time position responsible for managing incoming patient calls, conducting phone-based assessments, and facilitating communication between patients and healthcare teams.
Key Responsibilities
- Answer and manage incoming calls professionally and courteously
- Conduct thorough assessments of patients' health concerns over the phone
- Relay messages and information between patients and healthcare teams
Required Qualifications
- High School Diploma or equivalent
- Exceptional customer service skills
- Ability to maintain confidentiality and handle sensitive information
- Proficient in using computer systems and electronic health records
- Previous experience in a medical office or healthcare setting is preferred
