Job Summary

Patient Navigator is a full-time position responsible for managing incoming patient calls, conducting phone-based assessments, and facilitating communication between patients and healthcare teams.

Key Responsibilities
  • Answer and manage incoming calls professionally and courteously
  • Conduct thorough assessments of patients' health concerns over the phone
  • Relay messages and information between patients and healthcare teams
Required Qualifications
  • High School Diploma or equivalent
  • Exceptional customer service skills
  • Ability to maintain confidentiality and handle sensitive information
  • Proficient in using computer systems and electronic health records
  • Previous experience in a medical office or healthcare setting is preferred
