Patient Navigator

Job Summary

To support a multi-disciplinary healthcare team, the full-time Patient Navigator will manage patient care through the healthcare system, establish relationships with patients, and coordinate appointments while working remotely.

Key responsibilities

• Serve as the primary point of contact for patients, helping them navigate healthcare services and remove barriers to care
• Collect and analyze market data to strategize payor population identification and verify insurance benefits for obesity treatment
• Educate providers and staff on payor regulations and maintain knowledge of pre-certification requirements for various healthcare services

Required qualifications

• Bachelor's degree in Healthcare Management, Public Health, or a related field; or an Associate's degree with two years of relevant experience; or a high school diploma with four years of relevant experience
• Three years of experience in the healthcare field, including one year of statistical data analysis
• Proficiency in Microsoft Office
