Patient Navigator

Job Summary

To support patients in overcoming barriers to treatment, the full-time Patient Navigator will assist with insurance navigation, financial assistance, and transportation resources in a hybrid work environment.

Key responsibilities

• Assist patients with Medicaid enrollment and navigate insurance options to ensure continuous access to treatment
• Educate patients on financial assistance programs and help complete necessary applications for support
• Connect patients with transportation resources and community services to reduce barriers to care and improve engagement

Required qualifications

• Associate's degree in Human Services, Healthcare Administration, Social Work, Psychology, Public Health, or a related field (preferred)
• Experience in healthcare, patient services, or community resources (preferred)
• Proficiency in insurance eligibility verification and benefit navigation (required)
• Ability to determine patient financial responsibility for services, including familiarity with Medicaid and Medicare (required)
• Demonstrated ability to build trust and rapport with patients
