Job Summary
A company is looking for a Patient Navigator.
Key Responsibilities:
- Conduct onboarding calls and assess patient needs
- Schedule follow-up calls and provide continued patient outreach
- Act as a liaison for 3rd party services and facilitate support
Required Qualifications:
- Two-year associate's degree or equivalent combination of experience and education
- Two years of directly related and progressively responsible experience preferred
- Experience in fields such as social work, nursing, counseling, or psychology
- Proficient in Microsoft Excel, Outlook, and Word
- Strong analytical and organizational skills
