Job Summary
To support the colorectal cancer screening program, the full-time Patient Navigator - New York Licensed will oversee a caseload of patients, acting as the primary liaison to facilitate colonoscopy scheduling and completion while working in a flexible hybrid schedule.
Key responsibilities
- Schedule and navigate direct access colonoscopies for patients referred to The CHOICE Program
- Establish relationships with patients to ensure commitment to completing their colonoscopy and address any questions or concerns
- Train and mentor less experienced Patient Navigators while tracking patient encounters in the CHOICE tracking database
Required qualifications
- Bachelor's degree preferred or High School Degree with relevant work experience
- Minimum of 2 years experience, with 3-5 years highly preferred
- Experience as a Patient Navigator or in a similar position
- Bilingual in Spanish preferred
- Experience with EMR systems, preferably EPIC
