Patient Navigator - New York Licensed

Job Summary

To support the colorectal cancer screening program, the full-time Patient Navigator - New York Licensed will oversee a caseload of patients, acting as the primary liaison to facilitate colonoscopy scheduling and completion while working in a flexible hybrid schedule.

Key responsibilities

• Schedule and navigate direct access colonoscopies for patients referred to The CHOICE Program
• Establish relationships with patients to ensure commitment to completing their colonoscopy and address any questions or concerns
• Train and mentor less experienced Patient Navigators while tracking patient encounters in the CHOICE tracking database

Required qualifications

• Bachelor's degree preferred or High School Degree with relevant work experience
• Minimum of 2 years experience, with 3-5 years highly preferred
• Experience as a Patient Navigator or in a similar position
• Bilingual in Spanish preferred
• Experience with EMR systems, preferably EPIC
