Patient Navigator, Palliative Care
Job Summary
Supporting the Bereavement Follow Up program, the PRN Patient Navigator, Palliative Care will assist patients in overcoming healthcare barriers while coordinating treatment plans and collaborating with healthcare teams, with a schedule that includes on-site and remote work as needed.
Key responsibilities
- Assist patients in anticipating, identifying, and overcoming barriers within the healthcare system
- Coordinate treatment plans by understanding patient diagnoses and consulting with healthcare teams
- Document interactions and outcomes while enhancing the patient navigation process based on community needs assessments
Required qualifications
- High school diploma or equivalent is required
- 0 to 2 years of relevant experience in a medical office or healthcare environment is preferred
- Proficiency in MS Office (Outlook, Excel, Word) or similar software is required
- Experience working with all levels within an organization is required
