Patient Navigator RN

Job Summary

A company is looking for a Patient Navigator (RN) - Serious Illness.

Key Responsibilities

• Build trusting relationships with patients and families facing serious illnesses
• Conduct comprehensive assessments and educate patients on their conditions and treatment options
• Coordinate care across the healthcare team and document navigation activities accurately

Required Qualifications

• Active RN license (multi-state compact preferred)
• 3+ years of clinical nursing experience, preferably in oncology, cardiology, or other serious illness specialties
• Deep clinical knowledge of disease-specific care and treatment pathways
• Experience in care coordination across multiple healthcare providers
• Familiarity with Medicare patients or managed care populations
