Job Summary

A company is looking for a Patient Outreach Specialist.

Key Responsibilities

• Manage and coordinate care for patient panels, ensuring quality and preventive wellness strategies
• Monitor patient attribution and coordinate preventative screenings and services
• Track quality measures, validate patient records, and communicate with care teams to support patient care plans

Required Qualifications

• Post-secondary education of one to two years in healthcare or a medical-related field is preferred
• A minimum of one year of work experience in ambulatory care, community outreach, or patient support is preferred
• Training in Motivational Interviewing and/or Mental Health First Aid is preferred
• Background as a Certified Medical Assistant (CMA) is preferred
