Patient Outreach Specialist

Job Summary

A company is looking for a Patient Outreach Specialist to support patient care and wellness strategies.

Key Responsibilities

• Manage and coordinate care for patient panels, monitoring attribution and identifying gaps in care
• Coordinate preventative screenings and wellness strategies, ensuring optimal health outcomes
• Collect and communicate patient data to support care team efforts and follow through on care plans

Required Qualifications

• Post-secondary education of one to two years in a healthcare or medical-related field preferred
• Minimum of one year of experience in ambulatory care, community outreach, or patient support preferred
• Training in Motivational Interviewing and/or Mental Health First Aid preferred
• Background as a Certified Medical Assistant (CMA) is preferred
