Job Summary
To support a clinic's integrated primary care efforts, the full-time Patient Outreach Specialist will manage and coordinate care for a panel of patients, ensuring quality patient care through preventive services and outreach, all while working remotely.
Key responsibilities
- Manage patient panels by monitoring attribution and coordinating wellness screenings and preventive services
- Track and report quality measures, ensuring the accuracy and integrity of patient records
- Collect data to identify barriers to care and communicate with the care team to support patient health plans
Required qualifications
- Post-secondary education of one to two years in a healthcare or medical-related field is preferred
- A minimum of one year of work experience in ambulatory care, community outreach, or patient support is preferred
- Training in Motivational Interviewing and/or Mental Health First Aid is preferred
- Background as a Certified Medical Assistant (CMA) is preferred
