Job Summary
To support integrated primary care, the full-time Patient Outreach Specialist will manage and coordinate care for patient panels, monitor attribution, and facilitate outreach to ensure optimal health and wellbeing, working remotely from various locations.
Key responsibilities
- Coordinate preventative screenings and wellness strategies for patient panels
- Monitor patient attribution and manage chronic disease registries to promote quality initiatives
- Collect and communicate patient data to identify barriers to care and support the healthcare team
Required qualifications
- Post-secondary education of one to two years in a healthcare or medical-related field preferred
- Minimum of one year experience in ambulatory care, community outreach, or patient support preferred
- Training in Motivational Interviewing and/or Mental Health First Aid preferred
- Background as a Certified Medical Assistant (CMA) is preferred
