Key Responsibilities

• Accurately enter patient referral details into the management system, ensuring compliance with HIPAA standards
• Verify insurance eligibility and process prior authorizations, escalating cases for clinical review when necessary
• Communicate referral outcomes to patients and referral sources, providing clear and empathetic updates
• Coordinate with home health agencies to ensure timely and appropriate patient care services

Qualifications and Skills

• High School Diploma or GED required; Medical Assistant or CNA certification preferred
• Minimum 2 years of experience in a medical setting or 5 years equivalent experience; home health background is a plus
• Proficient in Microsoft Office and comfortable navigating multiple computer systems
