Patient Resource Specialist

Job Summary

A company is looking for a Patient Resource Specialist (Remote).

Key Responsibilities

• Act as a liaison between patients and community resources to ensure timely access to essential services
• Collaborate with care managers and healthcare providers to address patients' social and financial needs
• Assist patients in applying for Medicaid and connecting them with relevant social services

Required Qualifications

• High School Diploma or equivalent required; college degree in Social Work or related field preferred
• At least 2 years of experience in case management, social work, or customer service
• Strong understanding of social determinants of health and their impact on patient outcomes
• Experience with public assistance programs, including Medicaid, is a plus
• Proficient with technology, including Google tools and various digital platforms
