Job Summary
Serving as the primary point of contact for patients and healthcare providers, the full-time Patient Support Case Manager will provide support throughout the patient journey, including pre and post prescription processes, while working remotely from Monday to Friday, 11:30am to 8:00pm EST.
Key responsibilities
- Build and maintain relationships with patients, caregivers, and healthcare providers to facilitate access to therapy
- Act as the main resource for incoming inquiries, providing timely updates to stakeholders on patient cases
- Educate patients on their treatment journey using program-approved materials and resources
Required qualifications
- Minimum of 5 years of pharmacy experience required
- Associate degree or equivalent program in pharmacy or healthcare preferred
- Desired certifications include PACS, CHES, or CCM
- Minimum of 2 years of patient support program experience preferred
- Pharmacy Technician certification is a plus
