Job Summary

To support patients battling cancer and rare diseases, the full-time Patient Support Case Manager will serve as the primary contact for patients and healthcare providers, guiding them through the patient journey while working remotely from 8:30 am to 5:00 pm AZ Time, Monday to Friday.

Key responsibilities:
  • Act as the main resource for patients, caregivers, and healthcare providers, identifying and navigating barriers to therapy
  • Build and maintain relationships with patients and healthcare teams to facilitate access to and continuation of therapy
  • Provide educational resources and support throughout the treatment journey, including managing inbound and outbound calls
Required qualifications:
  • Minimum of 5 years of pharmacy experience required
  • Desired: Associate degree or equivalent from a two-year college or technical school in pharmacy or healthcare
  • Desired: Certification as a Pharmacy Technician, PACS, CHES, or CCM in healthcare or social science
  • Minimum of 2 years of experience in a patient support program preferred
