Job Summary

A company is looking for a Patient Support Case Manager to join their organization.

Key Responsibilities
  • Act as the primary point of contact for patients, caregivers, and healthcare providers throughout the patient journey
  • Identify barriers to therapy and assist patients in navigating these challenges to maintain prescribed treatment
  • Provide educational resources and support via inbound and outbound calls to ensure a successful patient experience
Required Qualifications, Training, and Education
  • Minimum of 5 years of pharmacy experience required
  • Associate degree or equivalent program in pharmacy or healthcare preferred
  • Desired certifications include PACS, CHES, or CCM in healthcare or social science
  • Minimum of 2 years of patient support program experience preferred
  • Pharmacy Technician certification is desired
