Job Summary

To support patient care remotely, the full-time PCM Care Coordinator will manage patient data communication, assist clinicians with medical information gathering, and engage with patients and families while working within EST hours.

Key responsibilities
  • Assist clinicians in gathering medical information for patients between visits using electronic health record systems
  • Monitor and report any medical concerns monthly, including falls, medication issues, and hospitalizations
  • Complete PCM interventions for approximately 15 patients per day and report issues to the ancillary program manager or Chief Clinical Officer as needed
Required qualifications
  • Minimum High School Diploma required
  • Basic knowledge of medical terminology and healthcare technologies, including electronic health record systems
  • Proven organizational skills and ability to follow up effectively
  • Experience with Microsoft Outlook, Microsoft Excel, and internet/cloud-based applications
  • Must have a quiet workspace free of distractions with internet access for remote work
