PCM Care Coordinator

Job Summary

To support the delivery of behavioral health care, the full-time PCM Care Coordinator will perform remote monitoring and principal care management for patients, engaging with medical teams and families while managing approximately 15 patients daily.

Key responsibilities

• Assist clinicians in gathering medical information for patients using various electronic health record systems
• Report monthly medical concerns, including falls, medication issues, and hospitalizations
• Complete PCM interventions as assigned and support training for new employees

Required qualifications

• Minimum High School Diploma required
• Basic knowledge of medical terminology and healthcare technologies, including electronic health record systems
• Proven organizational skills and ability to follow up effectively
• Must have a quiet workspace with reliable internet for remote work
• Excellent written and verbal communication skills
