Job Summary

A company is looking for a Remote In-Home Services Case Manager, Pediatrics.

Key Responsibilities

• Manage a case load of clients aged 4 to 18, providing case management services in community settings
• Assist in implementing treatment plans and develop crisis plans and strategies for success in various environments
• Connect families to community resources and provide 24/7 on-call availability

Required Qualifications, Training, and Education

• Bachelor's degree in Social Work, Psychology, or Counseling
• Minimum of 1 year of applicable experience
• Basic Life Support certification
• Valid driver's license with a clean driving record
• Applicable vehicle insurance
