Job Summary

To support a growing pediatric practice, the part-time hourly Resource Pediatric Registered Nurse will provide real-time clinical support and triage for newly re-engaged patients, ensuring timely access to care and collaboration with interdisciplinary teams in a remote setting.

Key responsibilities:

• Provide real-time clinical support and triage for newly re-engaged patients
• Deliver proactive and compassionate care as the first point of contact for patients and caregivers
• Collaborate with interdisciplinary team members to develop and execute individualized care plans

Required qualifications:

• Licensed RN in at least one state with eligibility for other state licensures
• Bachelor's degree in nursing from an accredited university preferred
• Minimum 1 year of care coordination or case management experience required
• Familiarity with Medicaid regulations and services required
• Bilingual in Spanish is required
