Peer Navigator

Job Summary

A company is looking for a Peer Navigator to provide support to trauma survivors in their recovery journey.

Key Responsibilities

• Serve as the primary care navigator for assigned patients, guiding them through all phases of care
• Conduct individual and group peer mentor sessions to empower patients in their recovery
• Coordinate care between healthcare providers and manage referrals to ensure patient needs are met

Required Qualifications

• Lived experience with accessing mental health treatment
• Fluency with PTSD or related experiences
• Training as a peer mentor (training can be provided for preferred candidates)
• Healthcare coordination experience preferred
• High School degree or higher; bilingual in Spanish is a plus
