Peer Support Specialist

Job Summary

To support a growing healthcare initiative, the part-time Peer Support Specialist will engage patients remotely to navigate health needs, coordinate care, and connect them with community resources while fostering trusting relationships to enhance health outcomes.

Key Responsibilities

• Building and maintaining trusting relationships with patients, families, and healthcare providers
• Engaging high-risk patient populations to address health questions and barriers to care
• Coordinating services and resources to support whole-person health and improve patient engagement

Required Qualifications

• At least 3 years of experience in patient navigation, community resource navigation, or care coordination
• Experience working with patients who have complex medical, behavioral health, or social needs
• Knowledge of medical terminology and primary care workflows
• Demonstrated ability to work effectively in a remote environment
• Familiarity with motivational interviewing or care navigation techniques
