Job Summary
Engaging patients remotely, the part-time Peer Support Specialist will build trusting relationships, navigate care systems, and connect individuals with community resources to enhance health outcomes.
Key responsibilities
- Building ongoing relationships with patients and coordinating care to address health needs and barriers
- Conducting assessments and providing education to support patient engagement and adherence to care plans
- Documenting interactions and collaborating with healthcare teams to ensure effective care navigation and quality performance
Required qualifications
- At least 3 years of experience in patient navigation, community resource navigation, or a similar role
- Experience working with patients who have complex medical, behavioral health, or social needs
- Knowledge of medical terminology, primary care workflows, and social drivers of health
- Demonstrated ability to work effectively in a remote environment
- Familiarity with motivational interviewing and care navigation techniques
