Pennsylvania and Tennessee Licensed Clinical Pharmacist

Job Summary

To support a growing healthcare team, the full-time remote Clinical Consulting Pharmacist, licensed in Pennsylvania and Tennessee, will provide clinical review and consultation on medication requests, evaluate therapy appropriateness, and collaborate with providers to ensure optimal patient care.

Key responsibilities

• Provide clinical review and consultation on medication requests, prior authorizations, and benefit determinations
• Evaluate therapy appropriateness, dosing, and safety based on evidence-based guidelines
• Collaborate with providers to clarify prescriptions and recommend alternatives when appropriate

Required qualifications

• R.Ph required; PharmD preferred
• Full, current, and unrestricted pharmacist license in PA and TN
• 1+ year of pharmacy experience preferred (retail, hospital, or managed care)
• Working knowledge of formulary management and clinical guidelines
• Commitment to evidence-based practice and continuous learning
