Job Summary

Providing outreach to patients with complex medical and social needs, the full-time Pennsylvania Certified Community Health Worker will assist in navigating the healthcare system, engage with patients through various methods, and collaborate with a multidisciplinary team to implement care plans, all while working remotely.

Key responsibilities

• Orients and assesses new Community Health Workers and monitors their proficiency during training
• Identifies and conducts outreach to medically complex patients, facilitating their connection to necessary resources
• Engages patients through phone calls, home visits, and telemedicine, while ensuring adherence to treatment plans

Required qualifications

• High School Diploma or Equivalent (GED) - Required
• Minimum of 1 year of related work experience - Required
• Certified Community Health Worker (CCHW) - Pennsylvania Certification Board - Required
