Job Summary

Providing outreach to patients with complex medical and social needs, the full-time Pennsylvania Certified Community Health Worker will assist in navigating the healthcare system, conduct assessments, and collaborate with multidisciplinary teams while working remotely.

Key responsibilities

• Orients and assesses new Community Health Workers while supporting their training and proficiency
• Identifies and engages medically complex patients through various outreach methods, including phone calls and home visits
• Facilitates care plan implementation and adherence by collaborating with the clinical team and conducting safety evaluations

Required qualifications

• High School Diploma or Equivalent (GED) required
• Minimum of 1 year of related work experience required
• Certified Community Health Worker (CCHW) certification from the Pennsylvania Certification Board required
