A company is looking for an Ambulatory Care Manager RN.

Key Responsibilities

• Coordinate patient care needs and monitor outcomes and service utilization
• Conduct telephonic outreach for clinical assessments and patient education
• Maintain communication with providers, practice staff, and payers to ensure continuity of care

Required Qualifications, Training, and Education

• Minimum of two years recent experience in telephonic care management outreach
• Preferred two or more years of recent home care experience
• Preferred two years recent insurance or case management experience
• ASN Degree required; Bachelor's Degree preferred
• RN License required
