Pennsylvania Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Case Manager in Behavioral Health (Crisis Triage) to provide remote support for at-risk members.

Key Responsibilities:

• Provides telephone triage, crisis intervention, and care navigation for at-risk members with mental health and/or substance use concerns
• Engages with members and families to assess needs and provide resources for timely access to behavioral health services
• Matches members with appropriate providers and coordinates care, ensuring effective communication among all parties involved

Qualifications:

• Active PA Registered Nurse License required; B.S.N. or LSW, LCSW, LPC preferred
• Minimum of three to five years of post-licensure behavioral health clinical experience
• Previous experience in Utilization Review or Case Management preferred
• Knowledge of DSM 5 and ASAM
• Ability to work independently and utilize critical thinking for complex issues
