Job Summary
Assisting patients and members with clinical support and education, the full-time Pennsylvania Licensed Care Connector will work remotely to resolve concerns, manage crisis situations, and advocate for individuals with special needs or social determinants of health.
Key responsibilities
- Serves as the primary contact for the Enhanced Member Supports Unit, addressing patient/member concerns and inquiries
- Assesses clinical issues to ensure accurate and prompt information is provided to patients, members, and providers
- Manages crisis situations and coordinates with internal departments to resolve issues effectively
Required qualifications
- Bachelor's Degree in a related field (Social Work, Healthcare Administration, Psychology, Public Health, Human Services, or similar)
- Minimum of 1 year of related work experience
