Job Summary

To support the Enhanced Member Supports Unit, the full-time Pennsylvania Licensed Care Connector will assist in resolving patient/member concerns, provide education and support regarding available services, and manage crisis situations while working remotely from Pennsylvania.

Key responsibilities
  • Serve as the primary contact for patients/members/providers, addressing clinical issues and concerns promptly
  • Assist patients/members with social determinants of health and navigate complex eligibility issues through effective communication
  • Collect and record data on caller needs and coordinate with internal departments to resolve issues efficiently
Required qualifications
  • Bachelor's Degree in a related field (Social Work, Healthcare Administration, Psychology, Public Health, Human Services, or similar)
  • Minimum of 1 year of related work experience
