Job Summary

A company is looking for a Care Management Coordinator RN (Acute UM) - Remote.

Key Responsibilities

• Oversee and coordinate medical care for members, ensuring appropriate and cost-effective treatment
• Evaluate treatment plans and establish the need for inpatient care, collaborating with physicians and facility staff
• Identify quality of care issues and refer cases to appropriate departments while maintaining accurate documentation

Required Qualifications

• Licensed registered nurse in PA; BSN preferred
• Minimum of three years clinical experience or equivalent, with a preference for medical management experience
• Active PA Licensed Registered Nurse required
• Familiarity with current trends in medical practice
• Proficiency in Microsoft Office applications
