Job Summary

A company is looking for a Care Manager to work remotely in the USA.

Key Responsibilities

• Completes comprehensive assessments and develops, monitors, and evaluates the member's Individualized Care Plan
• Participates in ICT rounds to provide updates on members' health status and concerns
• Acts as a liaison between the member and community-based agencies, coordinating care and monitoring benefit utilization

Required Qualifications

• Associate's degree required; Bachelor's degree preferred
• Licensed RN or Licensed Social Worker, with licensure in PA or through a compact license
• Minimum of one year of care management experience
