Job Summary

Key Responsibilities

• Oversees care management functions for a designated population
• Promotes patient self-management and provides education about disease processes
• Coordinates care for high-risk patients during transitions to or from care facilities

Required Qualifications

• Bachelor's Degree in Nursing or equivalent experience
• 5 years of experience in case management, disease management, or related settings
• 2 years of prior electronic medical record (EMR) experience
• Certification as a Registered Nurse in Pennsylvania or equivalent certifications
• Ability to recognize the impact of cultural, social, and economic factors on health services
