Job Summary
A company is looking for a Care Navigation Partner for Physician Practices.
Key Responsibilities
- Deliver education, outreach, and in-person assistance to patients
- Collaborate with stakeholders to triage and process patient calls and manage care plans
- Assist patients in applying for community resources and support practice quality efforts
Required Qualifications
- Bachelor's Degree in nursing
- 3 years of clinical experience in an inpatient or ambulatory setting
- American Heart Association Basic Life Support certification
- Licensed Registered Nurse in Pennsylvania upon hire
- Master's Degree in nursing is preferred
