Job Summary

A company is looking for a Care Navigator - Population Health.

Key Responsibilities

• Implements care plans tailored to patients' needs based on clinical assessments
• Coordinates follow-up care for patients not meeting clinical goals and manages documentation in the electronic health record
• Collaborates with multidisciplinary teams to provide education and navigation services for patients and families

Required Qualifications

• Bachelor's Degree in Nursing or equivalent experience
• 5 years of experience as a registered nurse (RN)
• 2 years of experience in case management, disease management, or related settings
• 2 years of electronic medical record (EMR) experience
• Licensed Registered Nurse in Pennsylvania upon hire
