Job Summary
Seeking a dedicated Pennsylvania Licensed Case Coordinator, the full-time remote position will coordinate care for clients in recovery, advocate for their needs, and facilitate connections to essential health and social services while ensuring effective and compassionate treatment.
Key responsibilities
- Coordinate client appointments, schedules, and treatment-related meetings while monitoring client progress
- Connect clients with community resources and ensure access to a full continuum of services
- Maintain accurate client records and documentation in compliance with federal, state, and accreditation standards
Required qualifications
- Master's degree in Social Work, Psychology, Counseling, Nursing, Chemical Dependency Studies, or a related field
- Addiction counseling certification preferred
- Experience in behavioral health, mental health, or substance use disorder treatment
- Experience facilitating individual, family, or process groups preferred
- Knowledge of state, federal, and accreditation requirements for behavioral healthcare services
