Job Summary
Collaborating with a multidisciplinary team, the full-time Pennsylvania Licensed Caseworker will coordinate care for mental health, behavioral, and social needs, facilitating communication among patients, families, and healthcare professionals while working remotely.
Key responsibilities
- Assist in the implementation of care plans and coordinate behavioral health and social services
- Evaluate psychiatric patients and provide crisis intervention, case management, and community outreach
- Act as a liaison between patients, families, and community agencies to address psychosocial needs and support treatment recommendations
Required qualifications
- Bachelor's Degree in Social Sciences
- Current Pennsylvania Licensed Caseworker certification
