Job Summary

A company is looking for a Clinical Pharmacist, Remote (PA/NJ/DE).

Key Responsibilities

Evaluate and optimize members' medication profiles and drug therapies
Educate members on medication use and address issues related to polypharmacy and medication adherence
Document interventions and monitor the effectiveness of medication plans

Required Qualifications

Doctor of Pharmacy or Bachelor of Science Degree in Pharmacy with 1-3 years of relevant clinical experience
Active State Pharmacy License
At least 5 years of practice experience or accredited Managed Care Residency
Current knowledge in clinical pharmacology and disease management
Proficiency in Microsoft Word, Excel, and Outlook
