Job Summary

A company is looking for a PA Licensed Clinician: LMSW, LCSW, or LPC in Behavioral Health.

Key Responsibilities

• Provide customized care and treatment plans for patients within assigned Care Pods
• Coordinate behavioral health care with primary care providers and monitor patient progress
• Document patient interactions and treatment recommendations in the electronic health record system

Required Qualifications, Training, and Education

• Master's degree in Social Work, Counseling, or a related field from an accredited program
• Licensed to practice in Pennsylvania as an associate clinician under supervision or as an independent clinician
• Experience with evidence-based treatment approaches
• Proficiency in behavioral health screening tools
• Technical proficiency in electronic health records and technology platforms
