Job Summary

Passionate about transforming healthcare, the full-time Pennsylvania Licensed Community Health Worker will empower patients, address social drivers of health, and collaborate with care teams to improve outcomes in a fully remote environment.

Key responsibilities

• Engage, educate, and coach patients and caregivers to navigate health systems effectively
• Connect patients with essential resources such as housing, food, and healthcare access
• Document patient encounters and support the development of patient care plans in collaboration with the care team

Required qualifications

• High school diploma or GED required; relevant lived experience is welcome
• Prior healthcare experience preferred, especially in geriatric care
• Proficient with Microsoft Office Suite and electronic medical records (EMR)
• Comfortable managing complex patients in a fast-paced, team-oriented environment
• Reside in the state of PA
