Pennsylvania Licensed Discharge Care Manager RN

Job Summary

To support high-risk members, the full-time Pennsylvania Licensed Discharge Care Manager RN will implement telephonic case management interventions, assess discharge needs, and coordinate care remotely while collaborating with clinical teams.

Key responsibilities:

• Evaluate member discharge needs and develop readmission prevention plans
• Coordinate care post-discharge with providers and members
• Document discharge activities and participate in team meetings for case discussions

Required qualifications:

• Bachelor of Science in Nursing
• Current, unrestricted Compact RN license in Pennsylvania
• 5+ years of recent experience in an acute inpatient hospital setting
• 2+ years of experience in discharge planning and/or case management
• Proficient in Microsoft Office Suite and capable of maintaining a secure high-speed internet connection
