Pennsylvania Licensed Health Care Manager

Job Summary

A company is looking for a Health Care Manager - Special Needs Plan (SNP) - Social Worker Preferred (Remote).

Key Responsibilities

• Ensure continuity and coordination of care for members with chronic conditions and complex health needs
• Develop and coordinate individualized treatment plans, evaluating their effectiveness and making necessary adjustments
• Monitor and evaluate treatment outcomes, collaborating with various departments to improve services

Required Qualifications

• Master's degree in a human service field or Licensed Pennsylvania RN with 6-8 years of clinical experience
• Five years of experience in clinical, care coordination, and/or case management
• General knowledge of best practices in health care, especially for special needs populations
• Computer proficiency and experience with Microsoft Office products
• Licensure in social services (LSW, LCSW, LPC) or as a Registered Nurse (RN) required
