Pennsylvania Licensed Health Care Manager

Job Summary

A company is looking for a Health Care Manager - Special Needs Plan (SNP) - Social Worker Preferred (Remote).

Key Responsibilities

• Ensure continuity and coordination of care for members with chronic conditions and complex health needs
• Develop and coordinate individualized treatment plans, monitor their effectiveness, and make necessary adjustments
• Collaborate with various teams to implement systematic revisions to improve service delivery

Required Qualifications

• Master's degree in a human service field or Licensed Pennsylvania RN with 6-8 years of clinical experience required; preference for Master's with LSW or LCSW
• Five years of experience in clinical, care coordination, and/or case management required
• General knowledge of best practices in health care, especially for special needs populations
• Computer proficiency required; experience with Microsoft Office preferred
• Licensure in relevant fields (e.g., RN, LSW, LCSW) and case management certification preferred
