Job Summary

A company is looking for a Remote Health Coach.

Key Responsibilities

• Serve as the primary nurse for members, providing guidance across all stages of health
• Engage members through coaching and case management to promote self-management and improve health outcomes
• Conduct assessments, develop individualized care plans, and monitor progress through ongoing telephonic follow-ups

Required Qualifications

• Active PA RN License or Compact RN License valid in Pennsylvania
• Minimum 3 years of experience in clinical practice, case management, or disease management
• Case Management Certification (CCM) preferred
• Strong clinical assessment and critical thinking skills
• Proficiency with computers and electronic documentation
