Job Summary
To support members in achieving their health goals, the part-time, contract Pennsylvania Licensed Health Coach will serve as the primary clinical resource for wellness promotion, chronic condition management, and care coordination in a fully remote capacity.
Key responsibilities
- Provide health coaching, case management, and care coordination services to support positive health outcomes
- Conduct comprehensive health assessments and develop individualized care plans with measurable goals
- Utilize motivational interviewing techniques to encourage behavior change and member engagement
Required qualifications
- Active Pennsylvania RN License or Compact RN License valid in Pennsylvania
- Minimum 3 years of experience in clinical nursing, case management, or disease/condition management
- Strong clinical assessment and critical thinking skills
- Proficiency with electronic documentation systems and basic computer applications
